November 29, 2016 Nathan Pless 5410 Lyons Rd., Apt. 104 Coconut Creek, FL 33073 Re: Lot Evaluation/ Lake View Estate/ Tax Map: A24-46 Dear Mr. Pless: A site and soil evaluation was conducted by the Person County Health Department at the property referenced above on November 22, 2016. The soil evaluation was done in accordance with the laws and rules governing wastewater systems in North Carolina (GS 130A-333, NCAC T15A: 18A .1900). Based on our evaluation, this property would be classified as Unsuitab[e for the installation of a septic system. The site is classified as Unsuitable for the following reasons: Rule .1940: Landscape Position/Topography: The majority of the lot is dissected by numerous gullies. Rule .1945: Available Space: There is insufficient area for the installation of a system. Due to the Unsuitable classification, a septic system permit cannot be issued for this property by the Person County Health Department, in accordance with State rules and regulations. You have the option of hiring a soil scientist to review this decision. A soil scientist has the option of proposing a system to serve the property that meets the requirements of Rule 1948(d). You also have the right to request an Informal Review of this decision by the State Regional Soil Scientist. A request for informal review must be made in writing to the local health department (form available). An informal review is conducted at no cost to you. !n addition, you have a right to pursue a formal appeal of our determination. To pursue a formal appeal, you must file a petition for a contested case hearing with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714.
